A growing Health Center in Rockland County is seeking a dedicated Behavioral Health Case Manager to support clients in accessing and coordinating essential mental health services. This role is ideal for someone who is compassionate, organized, and committed to helping clients navigate care, develop treatment plans, and connect with appropriate resources to support their overall well-being.

Responsibilities:
  • Complete health screening assessments
  • Conduct comprehensive behavioral health assessments to identify client needs
  • Develop individualized treatment plans in collaboration with clients
  • Provide referrals and coordinate related activities to secure appropriate services
  • Monitor client progress and update treatment plans as clinically indicated
  • Coordinate care among multiple providers to ensure integrated and cohesive treatment
  • Advocate for client access to services, benefits, and community resources
  • Provide psychoeducation to clients and families regarding mental health conditions and treatment options
  • Conduct follow-up outreach and engagement as needed
  • Assist with crisis intervention and safety planning when indicated
 
Qualifications:
  • Experience with referrals and obtaining services for clients.
  • Knowledge of relevant federal, state, and local regulations.
  • Bachelor's degree in social work, psychology, or a related field.
  • Ability to work independently and carry case load without constant monitoring.
  • Experience collaborating with a multidisciplinary team.
