Case Manager
Responsibilities:
- Complete health screening assessments
- Conduct comprehensive behavioral health assessments to identify client needs
- Develop individualized treatment plans in collaboration with clients
- Provide referrals and coordinate related activities to secure appropriate services
- Monitor client progress and update treatment plans as clinically indicated
- Coordinate care among multiple providers to ensure integrated and cohesive treatment
- Advocate for client access to services, benefits, and community resources
- Provide psychoeducation to clients and families regarding mental health conditions and treatment options
- Conduct follow-up outreach and engagement as needed
- Assist with crisis intervention and safety planning when indicated
Qualifications:
- Experience with referrals and obtaining services for clients.
- Knowledge of relevant federal, state, and local regulations.
- Bachelor's degree in social work, psychology, or a related field.
- Ability to work independently and carry case load without constant monitoring.
- Experience collaborating with a multidisciplinary team.
